• Completed Marshall Health Network Covid-19 Monoclonal Antibody Order Set (ALL areas must be completed by provider)
• Positive COVID-19 test results
• Demographics or face sheet
• Contact number for patient
• Insurance informatin

Once the checklist is completed, please fax information to 304.399.4664. A Hoops Outpatient Infusions member will contact the patient with an appointment time.
